The Arkansas Tech University Office of Human Resources, in conjunction with Saint Mary’s Regional Health System, is offering a flu shot clinic for ATU-Ozark employees on Tuesday, Oct. 16, in Student Services Conference Center room 117-B.
Flu shots are a free benefit available to ATU employees and their covered dependents on the Tech health plan. Dependent children must be 12 or older to receive the vaccine. The clinic will be available from 1-5 p.m. The consent form and vaccine information statement is located on the Office of Human Resources website at https://www.atu.edu/hr/forms.php.
Individuals who wish to receive a flu shot will need to bring:
• Completed influenza vaccine 2018-2019 consent form
• Arkansas Blue Cross Blue Shield insurance card
